Your Assessment
Your appointment will be carried out by a qualified Women’s Health Physiotherapist who will explain the process and answer any questions before starting.
The assessment will include a discussion about your symptoms and concerns. A physical examination may be recommended to help form an accurate diagnosis.
- All examinations will be clearly explained beforehand
- Your consent will be obtained before any assessment is carried out
- You may decline or stop any part of the assessment at any time
Occasionally, you may experience mild discomfort or a temporary increase in symptoms following assessment. Please let your physiotherapist know immediately if anything feels uncomfortable.
Comfort and Dignity
Your comfort, dignity and privacy are our priority. You may be asked to undress from the waist down so an appropriate examination can be carried out. If an internal examination is recommended, this will be fully explained and will only proceed with your
explicit consent. You will be covered throughout, given privacy to change, and you can stop at any time.

Your Treatment Plan
Following your assessment, your physiotherapist will discuss their findings and agree a personalised treatment plan with you. This may include:
- Advice and education
- Individual exercises or self-management strategies
- Hands-on treatment, where appropriate
Some treatments may cause temporary soreness or, occasionally, mild bruising. For certain specialised treatments, additional written consent may be required.
Chaperones
You are welcome to bring a chaperone to your appointment. Please let our reception team know in advance so this can be arranged. Patients under 18 must attend with a responsible adult.
Quality and Clinical Standards
To maintain the highest standards of care, patient records may be reviewed as part of routine clinical audit. These reviews are confidential, and records are not copied, removed or used for any other purpose.
Your Declaration
By submitting this form you confirm the following:
- I confirm I have read and understood the content of this consent form
- I consent to undertake an assessment and recommended treatment (if applicable)
- I consent to Peebles Physiotherapy Ltd storing my personal and medical information, during my referral period and for as long after discharge, as is required by law
- I consent to my physiotherapist sharing my personal data and medical information with my Insurance Company for the purpose of progressing my case (if applicable)
- I consent to Peebles Physiotherapy Ltd sharing relevant personal & medical information with other clinicians, including my GP (if appropriate)
- I understand that I am responsible for the cost of my treatment(s) and agree to pay a cancellation fee of 50% if appointments are cancelled within 24 hours of the appointment time, and a full fee for non-attendance of appointments
- I understand that I may withdraw my consent at any time, without prejudice
